Methods of payment:
Payment should be made in Euro by one of the following methods:

The payment transfer form should clearly state the name(s) of the delegate(s) and should clearly state: “6 th International Forum on Mood and Anxiety Disorders”

• By Bank Cheque - Cheque to be made in Euroto PUBLI CRÉATIONS and to be forwarded together with the Registration Form.

• By Credit Card - All major credit cards are accepted (Visa, Eurocard/Mastercard, American Express). Please enter the credit card number and expiration date in the appropriate space on the Registration Form.

• By Internet - is available an "on line" registration form with payment by credit card in security certificate mode.

Confirmation of Registration:
Invoice, which will serve as confirmation letter of registration, will be sent to all delegates upon receipt of the relevant payment. Cancellations and Refunds: Notification of cancellation of registration must be sent in writing to the Organizing Secretariat PUBLI CRÉATIONS (ifmad@publicreations.com).
If received by October 6th 2006, all fees will be refunded less 20%. No refunds will be made after date October 6th 2006. All amounts will be settled after the Forum.
