The payment transfer form should clearly state the name(s) of the delegate(s) and should clearly state: “7th International Forum on Mood and Anxiety Disorders”

• By Cheque in Euro - made out to PUBLI CRÉATIONS and to be forwarded together with the Registration Form.

• By Credit Card - All major credit cards are accepted (Visa, Eurocard/Mastercard, American Express). Please enter the credit card number and expiration date in the appropriate space on the Registration Form.

• By Internet - is available an "on line" registration form with payment by credit card in security certificate mode.

Confirmation of Registration:
Invoice, that will serve as confirmation letter of registration, will be sent to all delegates upon receipt of the relevant payment. Cancellations and Refunds: Notification of cancellation of registration must be sent in writing to the Organizing Secretariat Publi Créations (ifmad@publicreations.com).

If received by October 12th 2007, all fees will be refunded less 20%. No refunds will be made after date October 12th 2007. All amounts will be settled after the Forum.
