Registration and payment

Individual Registration Fee: Euro 350,00 + Euro 56,00 (VAT 16%)

Registration fee includes
• Attendance at all Scientific Sessions
• Opening Ceremony
• Welcome Reception
• Lunches and Refreshments during breaks
• Congress kit (delegate bag with name badge and final programme)
• Entrance to the Exhibition.

Confirmation of Registration
A receipted invoice, which will serve as confirmation of registration, will be sent to all delegates upon receipt of the relevant payment.

Cancellations and Refunds
Notification of cancellation of registration must be sent in writing to the Organizing Secretariat Publi Créations (rti@publicreations.com).
If received by December 15th, 2006, all fees will be refunded less 30%.
No refunds will be made after date December 15th, 2006.
All amounts will be settled after the Forum.

Payment should be made in Euro by one of the following methods:

The payment transfer form should clearly state the name(s) of the delegate(s) and should clearly state: "4th International Respiratory Forum on Tract Infections".
• By Bank Cheque - Cheque to be made in Euro to Publi Créations and to be
forwarded together with the Registration Form.
• By Credit Card - All major credit cards are accepted (Visa, Eurocard/Mastercard,
American Express). Please enter the credit card number and expiration date in
the appropriate space on the Registration Form.
• By Internet - is available an "on line" registration form with payment by credit card in security certificate mode.
